A Municipal Services Agreement must be signed by the property owner and submitted with any building permit application that includes plumbing. Jamaica Beach Municipal Services Agreement - required with any building permit application involving plumbing, such as new house construction and certain additions or remodeling projects. Building & Zoning Permit Application   - for all types of construction, installation, and repair work, except for the following: Beachfront Construction Permit Application - required for construction within 1,000 feet of the beach vegetation line. Water Department Application For Service - use this form to connect water service; form must be submitted in person along with identification and applicable fees (contact the Water Department for fees). Request for Voluntary Discontinuance or Disconnection - use this form to disconnect water service; form may be mailed or faxed. Transfer Request - use this form to transfer water service into renter's name; form must be submitted prior to renter submitting application for service; form may be mailed or faxed. Declaration of Rental Property - use this form to declare your residence as rental property; form may be mailed or faxed. Declaration Release Form - use this form to void the Declaration of Rental Property; form may be mailed or faxed.   Online Utility Payment    - use this link to pay your water bill online. Police Department Request for Copy of Accident Report - submit this form to request a copy of an accident report from the Jamaica Beach Police Department. Request for Periodic Check - use this form to advise the police when you are going to be away from your home and would like it to be placed on a "watch" list. Application For Employment Municipal Court Plea Form - can be used to submit various pleas and requests to the Jamaica Beach Municipal Court. Miscellaneous Permit For Use Of Park Facilities - grants exclusive use of pavilion and kitchen facilities. Public Information Request - use this form to submit a request for the release of Public Records. Report of Hotel Occupancy Tax - use to report and remit hotel occupancy taxes collected.
